Fawcett Collectors of America (FCA) #22 by C.C. Beck
The FCA is a long-running fanzine, which is now published as part of Alter-Ego (http://www.twomorrows.com/alterego/index.html). This interesting cover from 1981 is by Captain Marvel co-creator C.C. Beck.

The image area is about 10.5" x 13.5" on a 12" x 16" piece of very heavy board. Also included is a piece of paper with a note from Beck to the editor.

Condition: The board has aged somewhat and is a cream color. The shaded areas are done in Zip-A-Tone. It looks like there's a pattern in the Zip-A-Tone in the larger image below, but this is just a problem with my scanner. The Zip-A Tone is all simply dots. A good amount of white-out or white paint was used for effect to create the white spots in the Zip-A-Tone areas and below the grass. There's a slight crease in the center right of the piece (you can see it in the larger scan) and a small cut in the top left corner of the board (you can also see that in the scan), but that's very far from the art area.

Larger Scan (note that the weird effect in the lower-left Zip-A-Tone and the shading on the right side are due to my scanner and are not on the actual art).

Note from Beck to the editor. This is on a piece of aged paper the same size as the art (I've only scanned in the section with writing):
